Hey, what's up dudes? It's Pat and welcome back to another video. And I know I've done a lot of like real life videos lately. But I've been reading the comments and a lot of people actually are enjoying seeing what's going on in my life and connecting more with you guys on pretty much everything going on. Um, today's actually really important, guys. So, um, Jen, she is leaving us. She's gone for like, let me look at the date right here. She's gone until the second. That's a week from now. She said maybe she'll be able to record something with me tonight, but maybe not. So, there might be seven days without a
video with Jen. I wanted to let you guys know about that. Um, here's the big deal, right? Today's the 26th. In two days, it's Jen's birthday. I don't really get her anything, but that's where you guys come in. Okay? That's where you come in. All right? This is what we're going to do. We're going to do it. All I need you to do is go over Jen's Instagram and uh follow her Instagram. She has no idea. I'm going to do this. We're hoping I don't know if this is possible, though. I really don't know. If we could get her to 1 million, my birthday present will be the best birthday present ever. I'm just saying it right now. Uh, and it's a big birthday. I think she's turning 30. Um, so if we She doesn't Does she look 30?
Like loads really slow. I don't think so. Um, if we could somehow get her to 1 million. Oh my god, that would be so cool. She's been trying to hit that for so long on Instagram. Um, so I'm going to link that down below. It's like literally the first thing in the description. and then when she gets back, she'll be really happy and she'll um be in a video hopefully um the second or something like that. That's what I'm thinking at least. So, that's one of the things going on right now. So, apparently Jen is going away for um for her birthday. I don't even know where she's going. I didn't even ask actually.
She's going where? She's going somewhere. She's just not here. That's all I know. So, she's going to be gone for like a week or something like that and then she'll be back and videos will resume as normal. I'll try to come up with some video ideas so that you guys don't have no videos, which I think I've been doing pretty good on lately, but let me know down below. I've been trying pretty hard. Um, other things going on. Oh my god. So, I know if you guys Okay, so there's a lot of things. There's a lot of things, right? Okay. So, as you guys have heard me mention a few times in the last couple months, I have like the weirdest rash. And like in one
video, I put makeup on it. I was like, whatever, you know, 2020, makeup all over my male body, whatever I have to do. Um, but yeah, you can see it right here. It looks awful. I'm telling you right now, it does not look good. Um, somehow it's not affected my dating life. Apparently amazing personality. Maybe that's what it is. I don't know. Um, so basically, you know, they tried to fix it a couple times. I've been to a few doctors, right? Um, they gave me like steroids. They gave me all kinds of weird like pills and stuff, and I'm like, well, that I don't think that looks better. That definitely doesn't look any better right now. Um, so I went to one of the best hospitals today um in
the world actually. Um, which is nice because they probably do a better job. Man, was that pricey by the way. I was like u So, okay, we have some notes uh from this u appointment here. Let me tell you what happened though. Let me just tell you what happened. So, I went in knowing I had to get a biopsy, which is when they cut a piece of you off, I guess, or scrape it off. They do something. Then they send it in for testing. I was like, "How bad could that be?" What do they do? Take a little uh little razor just like and then it's fine. No, that's not what it was. That's not what it was at all.
Literally, what they were like, they're like, "Well, we're going to like, you know, give you a shot to numb it." I'm like, "Why? Why do we have to numb it?" I mean, how it's not going to hurt, is it? They're like, "Well, yes, yes, it's going to hurt." Um, I was like, "So, like, that's going to be real painful." And they're like, "You're going to have a couple stitches in every spot." Did they do it? They did it in two spots. One is on my side right here and one is on my leg. And I'm just going to prove it so I'm not lying. This is a weird position I'm in. Didn't expect this today. There we go. Right there. So, they cut me open right there. And
you can see there's like blood under it, too. It was horrifying. How could I possibly get back into this position? There we go. So, they cut it and like I panic so bad. I do not like blood. I don't even see blood. I'm not even looking. I'm laying down flat, right? I'm looking up. I'm like, my eyes are like closed and they're like, "You doing all right?" And I'm like, "No, I'm not doing all right." I'm not, this isn't a joke. This is a serious thing that happened to me today. I'm like, "No, I'm not doing okay. I don't feel very well." Um, and then like they finish like that leg and I'm like, "Okay, I'm going to puke." Um, like I felt like I was going to pass out and then that then I also
felt like I was going to puke at the same time, which is a weird combination. So they had to give me like a little bag and um they put me like almost upside down apparent because you won't pass out cuz the blood goes to your head. Weirdest position ever. Um and they put like wet towels on me and stuff. Then they continued and then they chopped the part off on the other side. Um and I was okay. Um I made it through all that. Then they did a photo shoot of me which was kind of cool. I went to a photo room. They took all these pictures. The lady was nervous there after they had told her that I was passing out. She's like, "He's not going to pass out on me, right? Someone want to stay with me?"
The lady, I'm like freaking this lady out. I'm like, "I'm fine now. If you're not cutting something off of me, we're probably going to get along pretty well." So, she cuts nothing off me. Very weird photo shoot. Really strange. Basically, I'm a model now, though. Um, pretty sure about that. Um, then I'm like, "Oh my god, I have to get blood taken, too." And if you do remember, I've talked about this before. Um, I had blood taken at one point, passed out, woke up, passed out again, could not be recovered at all, then took an ambulance to the hospital. So, I'm so nervous whenever I give blood. So, I go there.
Um, I the guy has such a strong accent, too. And I'm like trying to like I'm trying to talk to him. It's such a strong accent. I'm like, "Oh my god." Because if I talk like I can get my mind off of it and I'm like, "Oh my god, I don't even know what he's saying." Uh, so I told him, "I need to lay down." Cool enough, at this place they had beds. I got to lay in a bed and then they took five vials of blood. That's the most I've ever had taken before. So, pretty horrific day, honestly. Um, made it through it. I'm okay. Let's read and see what they said about me here. Okay. The rest started about two months ago on after an illness characterized by fever, sore throat, and GI symptoms. I don't
even know what that means. Um, CO testing was negative on, you know, it was negative then, it's not negative anymore. Um, as was screening for strep infection. We're not going to try to read strep taco or what trep tacos. I wouldn't eat one of those. Um, the rest started within three or 4 days of the fever on the upper arms, which is right here. Has not changed apparently. And spread across the trunk and down onto the legs. Snap out of my legs. Trust me, you just saw my leg. Didn't look bad. Um, it also remained persistent since that time. It has not responded to either uh systemic corticcoids or topical corttoids.
It is only minimally prick. This is like a whole another language right here. All right. Um there's no family history of psoriasis. The patient has a history of mild dandruff. Hey, just cuz I said they asked me about dandruff and I was like I don't know. I think I've seen some before. Now I have to tell the whole world like that I have dandruff. I don't know if I have it. I was unsure. I've seen it in my life at some point. Not like I don't like shake my head and snow comes out of it. But anyways, in the past, otherwise no problems with previous rashes. I recommended that he come in for a biopsy, but did not tell
that he would chop she would chop it off. I knew I thought it was like a scrape. I don't know. He reports that the rash is unchanged since the video visit last week. I had like one of those Zoom calls. Was so weird, right? And it froze like halfway through. So, her face was stuck in one place, but I just heard her voice and I just went with it. Um, past medical history, anxiety is what they learned today. Um, okay, let's go on here. Patient lives locally, creates YouTube videos professionally. They literally wrote that on my thing. I thought that was really funny. Uh, skin no other lesions or rashes of concern.
Constitutional. No associate no associated unexplained fevers or weight loss. If anything, I've put on five pounds. That's good, right? Um physical examination constitutional appears welldeveloped wellnourished in no apparent distress. I'm like help me. Uh neuro psychiatric alert and oriented pleasant effect and mood. I just got I just got flirted with by the doctor right here. Pretty sure she flirted with me through this pleasant effect/mood. I have a pleasant effect in mood. I appreciate that. Thank you. Patient is quite anxious about undergoing a biopsy today. I'm like, I don't feel good. Please stop. Eyes. No sclerural injection or ectctorus. Thank god I didn't want any of those. Um, no eyelid abnormalities. Cardio. No lower extremity edema. I have examined
the scalp face, neck, chest, abdomen, back, bilateral, upper extremities, and bilateral lower extremities. And I didn't have a lot of clothes on today. Be honest. See, that's why I'm getting the call. Um, okay. Let's see. Where are we? Involving the trunk, upper and lower extremities, the patient has multiple well- definfined annular earthatius scaly plaques, some of which are co elicited into confluent areas of ermma. Am I dying? I really don't know. Themma has a yellow red quality to it. No islands of sparring. Not sparring. It's not like No, I didn't even try to fight her. Not even once. No nails changes noted.
That's good. I don't want to have weird nails. I'll be I'd have to start painting them and like I'd have to like throw it off like I just paint my nails. Not a big deal. Umbilicus is involved. Is that my umbilical cord? Because I'm pretty sure that was removed as a baby. My umbilicus. I don't know. I don't have my umbilicus any umbilicus anymore. These my old um scaling of the scalp as well as the postural areas. No interglutium.
Pinking noted. That's good. Okay. So, let's see what's going to happen to me. You're probably wondering, my nose is getting itchy because I'm like struggling here. You're wondering what's going to actually happen to me, right? Differential diagnosis is broad. It includes psoriasis PRP. I don't even want to Google it, but you guys probably will. Um, we will proceed with two diagnostic biopsies today. In addition, we will undertake screening lab work including um CBC. That's like my complete blood count. Um which is just like the normal test and I got it back already. Um completely normal. Also, my liver testing was normal and my kidney testing was normal.
I'm extremely healthy other than having a red weirdness on my body. So, that's good. So, all that and apparently they did another strep thing to make sure I don't have strep throat, but I mean it was months ago. It is possible to keep it for that long. I guess we briefly talked about potential management of this eruption should it turn out to be consistent with psoriasis. We discussed phototherapy as an option as well as systemic therapies. Basically injections or like some light therapy. Light therapy sounds nicer um and probably is better for you. But it's three times a week for like a couple months and then it will be gone. Or they can inject me with some terrible stuff into me and
that will probably make it gone too. I'm not really sure what I'm going to choose yet, but apparently it's not a big deal and it will be gone in like a couple months. Um, I just had to go to a very good doctor and I had to um get it cut off me first so they can actually test it. Once we have the results of the biopsy would be we once Oh my god, I can't even speak. Co brain. Um, once we have results of the biopsy would be contact see is this even a normal sentence? Once we have results of the biopsy would be contact with the patient to further management. No, they made a mistake in their spellings. All right. Punch biopsy. She literally, this girl punched me so hard it literally knocked the skin off my body. That is what a
punch biopsy is. I recommended a punch biopsy for purposes of further evaluation diagnosis. The right flank. You know when you flank steak? My right flank's over here apparently. I can feel uh feel the band-aid on it. So they think it's psoriasis. Um, and skin biopsy by punch technique. That's who's doing this. One punch man. I don't understand. So, here we go. Procedure. No. Verbal consent was obtained and a time was performed. The patient was counseledled about risk of bleeding, scar, and infection. I'm pretty sure she didn't ask me. Um, if I she I'm pretty sure she didn't say that I would be bleeding, scarring, or infected. No one told me I was going to be bleeding, scarring, or infected. I mean, I understand that cutting me could do
that, but I didn't hear those words. There he was clean with an alcohol swab and they used some kind of lidocaine on me. Um, and then a five millimeter punch. Um, it's like the Bruce Lee's one punch kind of specimen. It was sent for pathology pathologic examination. Oh my god. Something blah. And they, you know, I recommended a punch bopsy for purposes of further evaluation diagnosis. So I guess it's like they could evaluate it better because of that. All right. This is just I don't even understand any of this stuff. Anyways, patient education. Let's talk about my brain here. Ready to learn? I like this. I like how they wrote this. No parent learning barriers were identified. Are you sure?
Okay. Whatever then. I don't know. Learning preferences included include listening. Yeah, she did speak to me and I did listen. Explain diagnosis and treatment plan. Patient guardian of patient expressed understanding of the content. I understood the patient was advised to follow up once pathology reviewed. Should the patient develop new unchanging on nonhealing lesions recommend FU sorry that's funny. Um FU either with uh PCP or dermatology. All questions were answered and the patient was counseledled to the best of my ability. Risk benefits and alternatives to treatment were discussed in detail. Um, and yeah, basically you guys are probably like, "How many bad things could have go on in such a short period
of time?" Well, you know what? 2020 is almost over and 2021's going to be amazing. Videos are back. Things are back. I feel pretty good. I look pretty bad, but what are you going to do? That's part of life. Um, go over to Jen's Instagram and definitely um, give it a follow. Um, I think she'd be really happy if she actually got to 1 million. Even if she was close, I think she'd be happy about it and really surprised cuz I'm not telling her I'm making this video and she never checks my channel. But uh yeah, guys, I am totally fine, by the way. Nothing to worry about. I feel
good. No doctors are worried they're going to do some weird things to me and like if they want just chop it all off. I'll do that. Why not? That would just be quicker anyways. But um yeah, guys, um Jen's not going to be around too much, but it'll be okay. I'll have some videos up. Until then, I love you dudes. Definitely subscribe. Maybe drop a like on this video and oh, ring the bell. That's important. Ring. and I'll see you guys later.
